Afin que nous puissions procéder aux différentes démarches entourant le décès de votre proche, voici un aide-mémoire des documents à apporter lors de la rencontre avec votre conseiller :

• Le numéro d’assurance sociale de la personne décédée et du conjoint survivant
• Le numéro d’assurance maladie de la personne décédée
• Quelques photos de la personne décédée
• La liste des membres de la famille et des proches à qui vous désirez transmettre l’avis de décès (facultatif)
• Les vêtements de la personne décédée, incluant des sous-vêtements et des bas
• Ses prothèses dentaires, ses lunettes, ses bijoux, son maquillage et autres objets (facultatif)
