To make your life easier, just follow these three steps and we'll take care of the rest.

1.  Contact the Æterna Funeral Complex at 514-228-1888 to schedule a meeting with a counsellor.

2. Inform the hospital, residence or care centre that you have placed the deceased in the hands of the Æterna Funeral Complex.

3.  Fill out the following form and send it to your advisor by clicking on "Submit".

Information about the deceased person
Email address(es) of other family member(s) who wish to be informed of the progress of the preparations relating to the person's death. (Separate the addresses with a comma if more than one.)
Spouse’s information

Information about the decision-maker

Information about the payer

In order for us to take the various steps related to the death of your loved one, here is a checklist of the documents to bring to the meeting with your advisor:

  • The social insurance number of the deceased and the surviving spouse
  • The deceased's health insurance number
  • A few pictures of the deceased person
  • A list of the relatives and close friends to whom you would like to send the obituary (optional)
  • The deceased's clothing, including underwear and stockings
  • The deceased's dentures, if any, glasses, jewellery, make-up and other objects (optional)
