The deceased's health insurance number
A few pictures of the deceased person
A list of the relatives and close friends to whom you would like to send the obituary (optional)
The deceased's clothing, including underwear and stockings
The deceased's dentures, if any, glasses, jewellery, make-up and other objects (optional)
